This form must be submitted to certify that aboveground and underground storage tanks which previously contained hazardous materials, and which are cut and/or cleaned on-site, have been properly cleaned and rendered non-hazardous. (Note: Submittal of this form is not required if the tank is not cut on-site and is merely rinsed to lessen the risk of explosion during removal and/or transportation off-site.)
